While there, make sure your
medical history and information is up to date. Please include your birthdate (year first) as well as current address. All consents and waiver forms need to
signed at time of booking.

Inside the portal you will enter your patient information and history. You will need to review and sign all waiver and consent forms at time of booking.
You will also book your appointment and find pre/post treatment instructions in addition to lots of other helpful information.
Conflict in your calendar? No worries, simply reschedule online.

Once inside the portal, double click on the appointment in question and adjust accordingly.
Cancellations/ reschedules/no shows within 48 hours of your appointment time will incur a $100 fee per 30 min booked. Policy below.
Cancellation/Late Reschedule/No Show Policy: I get it! Life happens, emergencies occur. Please know we do everything we can to accommodate your schedule, we trust we mutually respect one another's time. If for any reason you cannot make your appointment, please change it via your patient portal (available to you 24/7 from your phone or computer; link above). Appointments canceled, rescheduled or adjusted within 48 hours or the original appointment time will incur a $100 late fee per 30 minutes booked, no exceptions. No-shows will be charged $100 via on-file credit card , invoice will be sent for any remaining balance and will need to be paid in full before rebooking considering the time lost. We count on you! Please don't leave us with space we cannot fill last minute :)

Payment: Credit card on file is required to hold appointment time but is not used for service payment. Payment for services is due at time of visit. Cash, Checks and major credit cards accepted. We do not accept CareCredit or Cherry. Thank you for your understanding!
All Sales are final. In clinic credit only for any discontinuation of package/bundle prepaid services.
