a date/time we can match for a consult or your sprucing up!

While there, make sure your

medical history and information is up to date. Please include your birthdate (year first) as well as current address. All  consents and waiver forms  need to 

signed at time of booking.

private patient portal. 

Inside the portal you will enter your patient information and history.   You will need to  review and sign all waiver and consent forms at time of booking.

You will also book your appointment and find pre/post treatment instructions in addition to lots of other helpful information.

Appointment Rescheduling

Conflict in your calendar? No worries,  simply reschedule online.

(available 24 hours a day from phone or computer. 

Once inside the portal, double click on the appointment in question and adjust accordingly. 

Cancellations/ reschedules/no shows within 24 hours of your appointment time  will incur a $100 fee.   Policy below. 

Cancellation/Late Reschedule/No Show Policy:  I get it!  Life happens, emergencies occur.  Please know I do everything I can to accommodate your schedule, I trust we mutually respect one another's time.  If for any reason you cannot make your appointment, please let me know at your earliest convenience or change via your patient portal (available to you anytime from your phone or computer).  Appointments canceled or rescheduled within 48 hours or the original appointment time will incur a $100 late fee. No-shows will be charged $100 via on-file credit card or invoice and will need to be paid in full before rebooking considering the time lost.  I count on you!  Please don't leave me with space I cannot fill last minute -- or I'll be lonely. :)
